Job Description:

A company is looking for a Telephonic Nurse Care Manager.

Key Responsibilities

Manage telephonic care for patients with chronic conditions and develop nursing care plans
Conduct assessments, evaluate data, and participate in quality improvement efforts to enhance clinical outcomes
Engage patients in their care using Motivational Interviewing techniques and document care accurately

Required Qualifications, Training, and Education

Nursing degree from an accredited college or university
Minimum of 5 years of broad clinical experience
At least 3 years of experience in home care, care coordination, or related fields
Compact license and/or CA license required
Strong computer skills and ability to navigate multiple programs
